At 1 y after injection, changes of glandular diameter were measured by tapeline to identify the net increase of maximal glandular circumference after augmentation of glans penis. Patient’s subjective visual estimation of glandular size was requested to assess the residual volume of hyaluronic acid gel. The patients estimated the visual analogue scale from Grade (Gr) 0 to Gr 4: Gr 0, no residual volume; Gr 1, less than 25% of initial volume; Gr 2, less than 50%; Gr 3, less than 75%; Gr 4, more than 75% or nearly same as initial volume. Patient’s satisfaction was also evaluated from Gr 0 to Gr 4: Gr 0, very dissatisfied; Gr 1, moderately dissatisfied; Gr 2, about equally satisfied and dissatisfied, Gr 3; moderately satisfied; Gr 4, very satisfied. Any adverse reactions were also evaluated.
The mean age of patients was 42.2 (30–70) y in Group I and 42.13 (28-61) y in Group II. (Table 1). The maximal glandular circumference was significantly increased compared to basal circumference of 9.13plusminus0.64 cm in Group I (P<0.01) and 9.49plusminus1.05 cm in Group II (P<0.01) at 1 y after injection. The net increase of maximal glandular circumference after glans augmentation was 14.93plusminus0.80 mm in Group I and 14.78plusminus0.89 mm in Group II. (Table 1, Figure 2). There was no significant difference between both the groups. In patient's visual estimation of glandular volume after augmentation, Gr 3 (more than 50% of injected volume) and Gr 4 (more than 75% of injected volume) was 38, 57% in Group I and 29.9, 70.1% in Group II. (Figures 3 and 4). Mean grade of visual estimation was significantly high in Group II (P=0.01). The percentage of postoperative satisfaction (Gr 3, 4) was 77% in Group 1 and 69% in Group II. The mean grade of patient's satisfaction was significantly high in Group I (P=0.003). There was no abnormal reaction in area feeling, texture and color. In most cases, initial discoloration by glandular swelling recovered to normal within 2 weeks. Postoperative consistency of glans penis was natural without deformity and maintained through 1 y. There were no signs of inflammation and no serious adverse reactions in all cases.
Injectable soft-tissue substitutes provide an affordable, nonsurgical alternative for correcting contour defects and soft-tissue augmentation. Several materials have been used for this purpose, including paraffin, silicone, and collagen5,6 Paraffin and silicone create intense foreign body reactions and are known to migrate from injection sites. Collagen, including rapid degradation, necessitates frequent reinjection and infrequent but significant hypersensitivity reactions.7 In recent years, implant materials have also been found to migrate to the lung and the brain.8
It is therefore advantageous to use degradable materials. The ideal filling substance for soft-tissue augmentation should be biocompatible, nonantigenic, nonpyrogenic, noninflammatory, nontoxic, easy to use, stable after injection, nonmigratory, long-lasting but reabsorbable, natural looking, and not too expensive.9,10 A ubiquitous component of all mammalian connective tissue, hyaluronic acid (Hyaluronan), is a naturally occurring polysaccharide, in the same chemical and molecular composition in all species; in the intercellular matrix of dermal layers of the skin of all species; therefore, it is highly biocompatible to use animal sources in humans without creating foreign body reactions.11,12,13 The material used in this study is based on hyaluronic acid, which has already been used in its native form as an implant for more than 20 y and in millions of individuals without causing adverse reactions. In this study, there were no serious adverse reactions in all cases.
Although the efficacy of hyaluronic acid was proved in various fields, the existence of potential space, technical feasibility, and long-term residence should be identified to use injectable hylauronic acid gel in the augmentation of glans penis. Previously, we reported the feasibility of glans penis augmentation by injectable hyaluronic acid in an animal experiment. In our study, hyaluronic acid gel was easily injected into the Beagle dogs via 27 G needle for elastic glans and showed long-term residence in the lamina propria. In this human study, it was not so difficult to inject hyaluronic acid into the dermis of glans penis. The nature of human glans was elastic and we developed the Fan technique. Most surgeons are already familiar with this technique, which is frequently used to make subcutaneous bulla for skin test of hypersensitivity and for easy dissection of subcutaneous tissues. In our animal study, we already revealed the potential space of lamina propria in glans penis. Although the long-term residual volumes were not measured, the implants were well maintained until 1 y in this study. We used a five-grade scale system. For more accurate estimation of glandular volume, a 10-grade scale may be useful, but it is more demanding for patients. Through the five-grade scale, the patient's self-estimation of long-term residence was fairly good in both the groups. The slow digestion of this gel shows that stabilization of the material through cross-linkage is able to increase its longevity several hundred folds compared to the natural polymer, without decreased biocompatibility. The implant has a property of degradation, but has a characteristic of isovolemic degradation. The isovolemic degradation keeps the gel always in balance with water in the tissue, and this increased capacity to bind water of a less concentrated hyaluronan network allows maintaining the correction even in low presence of the materials. Another advantage is easy supplementation by reinjection in cases of long-term volume loss. Like other fields of soft-tissue augmentation, there was no serious adverse reaction in this study. There was no abnormal reaction in area feeling, texture, and color. In most cases, initial discoloration by glandular swelling recovered to normal within 2 weeks. In most patients, local application of an anesthetic cream was sufficient, but a few presented penile pains.
The demand for penile augmentation continues to increase as the media exposes normal male figures and advertisements create interest in corrective surgery. Penile augmentation is a necessary procedure for such patients: psychogenic impotence or depression for their small penis, congenital micropenis, and penile reconstruction. Current penile augmentation with a dermofat graft alone may produce an iatrogenic penile deformity of thick penile body with relatively small glans. Unlike the Western country, augmentation phalloplasty (dermofat graft for girth enhancement and/or penile lengthening) is very popular for subjective small penis in Asian countries. However, the postoperative patient's satisfaction is not so good. All the Group I patients were not small glans penis only but were composed of small glans penis, small penis, and some early ejaculation patients. After thorough information about phalloplasty, Group I patients consented to only glans penis augmentation instead of total penile enhancement. In this study, postoperative glandular volume was greater in Group II than in Group I. Despite greater volume, patient's satisfaction was higher in Group I of subjective small glans penis. It may be the result of increased self-esteem and decreased anxiety after augmentation. In Group II, a combination of dermofat graft with glandular enhancement by injectable hyaluronic acid gel could make a well-looking penis.
In human glans penis, injecting hyaluronic acid gel into the dermis was not so difficult and the implants showed long-term residence in the patient's visual estimation and resulted in high satisfaction rate of patients. These results suggest that injectable hyaluronic acid gel is a safe and effective material for soft-tissue augmentation in patients with small glans penis. Long-term efficacy of more than 1 y needs to be demonstrated.

Is penis enlargement possible?

Growth hormone (GH), also known as somatotropin, is a peptide hormone produced by the anterior lobe of the pituitary gland. Growth hormone secretion occurs in a pulsatile fashion following a circadian (daily) rhythm, which is controlled by a central area of the brain known as the hypothalamus. I have been using HGH for almost 10 years, on and off. Both on and off cycle. Although augmentation phalloplasty is not an established procedure, some patients still need enlargement of their penis. Current penile augmentation is girth enhancement of penile body by dermofat graft. We performed this study to identify the efficacy and the patient’s satisfaction of human glans penis augmentation with injectable hyaluronic acid gel. In 100 patients of subjective small penis (Group I) and 87 patients of small glans after dermofat graft (Group II), 2 cm3 of hyaluronic acid gel was injected into the glans penis, subcutaneously. At 1 y after injection, changes of glandular diameter were measured by tapeline. Patient’s visual estimation of glandular size (Gr 0–4) and patient’s satisfaction (Grade (Gr) 0–4) were evaluated, respectively. Any adverse reactions were also evaluated. The mean age of patients was 42.2 (30–70) y in Group I and 42.13 (28–61) y in Group II. The maximal glandular circumference was significantly increased compared to basal circumference of 9.13plusminus0.64 cm in Group I (P<0.01) and 9.49plusminus1.05 cm in Group II (P<0.01) at 1 y after injection. Net increase of maximal glandular circumference after glans augmentation was 14.93plusminus0.80 mm in Group I and 14.78plusminus0.89 mm in Group II. In patient's visual estimation, more than 50% of injected volume was maintained in 95% of Group 1 and 100% of Group II. The percentage of postoperative satisfaction (Gr 4, 5) was 77% in Group 1 and 69% in Group II. There was no abnormal reaction in area feeling, texture, and color. In most cases, initial discoloration by glandular swelling recovered to normal within 2 weeks. There were no signs of inflammation and no serious adverse reactions in all cases. These results suggest that injectable hyaluronic acid gel is a safe and effective material for augmentation of glans penis.
Although augmentation phalloplasty is not an established procedure, some patients still need enlargment of their penis. Current main procedures for augmentation phalloplasty are penile lengthening and girth enhancement by dermofat graft. There is no established procedure for glans penis augmentation. The limitations of glans penis augmentation are poor understanding of glans penis anatomy, technical shortness, and lack of adequate material. In the last decade, hyaluronic acid has been shown to possess many properties that suggest its value in several medical applications, particularly in ophthalmology, orthopedics, and soft-tissue augmentation with proven efficacy and safety.1,2,3 Recently, we also reported the feasibility of injectable hyaluronic acid gel in augmentation of glans penis in vivo.4
We performed this study to identify the efficacy of injectable hyaluronic acid gel in augmentation of the human glans penis.
Patients
A total of 187 patients were recruited to this study. Patients of Group I were not satisfied with their penis, had a sense of inferiority because of their small penis and suffered from sexual dysfunction owing to decreased self-esteem. In all, 87 patients of Group II were dissatisfied with their relative small glans after augmentation phalloplasty with girth enhancement by dermofat graft. In both groups, 2 cm3 of hyaluronic acid gel was injected into the glans penis, subcutaneously.
Injection method
Under local anesthesia, 30 min after topical application of anesthetic cream Emla® (lidocaine 25 mg, prilocaine 25 mg, Astra Xeneca), 2 cm3 of injectable hyaluronic acid gel (Perlane®, Q-med, Uppsala, Sweden) was injected via a 27 G needle. The injection needle was indwelled subcutaneously at proximal one-third from tip of glans to coronal sulcus; thereafter hyaluronic acid gel was injected by the Fan technique (Figure 1). After injection of Perlane®, the undulation of glandular surface was supplemented by injection of Restylane® (Hyaluronic acid gel, Q-med, Uppsala, Sweden) via a 30 G needle. Both Restylane® and Perlane® are injectable hyaluronic acid gel and have the same composition of 20 mg/ml of stabilized hyaluronic acid gel. The difference between the products is the size of the gel particles. The molecular weight of hyaluronic acid in its pure form can be determined. However, hyaluronic acid in its pure form is not stabilized. Injectable hyaluronic acid gel is chemically modified hyaluronic acid product to increase its longevity in the tissue and to form a gel. It is not relevant to talk about molecular weight, as it cannot be determined for a stabilized gel. The approximate number of gel particles is 100 000/ml in Restylane® and 1000/ml in Perlane®. For this reason, Q-med recommends the 30 G needle to inject Restylane® into the mid- to upper part of the dermis and a 27 G needle to inject Perlane® into the deep layer of dermis.

Positive:
The dick feels more loose than before, it stretches much easier and longer without warmup.

Negative:

It seems like it is more veins than before also (harder to find good places to pin).
The dick looks a bit thicker in the middle (right side) when using extender. Don’t know if it has been site specific growth here but it doesn’t show when erect or flaccid so it doesn’t matter.
And a new thing. It has affected my wellbeing.. sometimes I feel really great and sometimes I just crash. I noticed that the crash comes after a few days without any injections.
This has also affected my booners… they have been more loose when I am in the “worse” periods.

I found this forum searching exactly for that, “intracavernosal IGF -1 for penile enlargement”. I have been extensively searching about that trough academic journals(since i have access to those articles that they don’t usually let you see it entirely) and what i have found is that the penis development is a really complex thing and it is modulated by several things.
Yet, all articles agree on some basic things, penile growth depend on growth factors(as IGF-1) which gives the penis the “potential”(that’s the word a few articles used”potential”) to grow, and androgen dependent factors which will make it grow till it reaches that potential(established by IGF-1 among other growth factors). Been more specific, as most of you probably know, the main androgen hormone involved in penile development is Dihydrotestosterone (DHT)
So my guess is that it would work even better if you provided some DHT together with your IGF-1.
At least, try using the supplement “creatine” which is cheap, easy to find, has no adverse effect and increases body natural levels of DHT.(a lot)
Maybe it speed up your results.
Another thing you should be worried about is PENILE FIBROSIS. It is a frequent condition among men who practice self intracavernosal injection treatments. Its caused by the frequent inflammatory process in your penis every time it needs to heal the injection site. It can get your penis curved, shorter and thinner!!!
Only the injection itself is a stressful enough to the penis to cause fibrosis it in some men. I believe you should avoid stretching in the same day you inject it to avoid extra inflammatory response.

Second of all, not once in my post did I say that I was going to be injecting any substances into my penis. Introduction
It is long known that the standard testosterone treatment applied on pre-puberty boys with a micro-penis yields no penile growth when given to adult men.
This is why adult males with a very small penis are left with little options (surgery) regarding their condition. However, this study shows that a different hormone approach may have the potential to induce penile growth after puberty.
In this study, a total of 20 males with idiopathic hypogonadotrophic hypogonadism participated. All of them presented with the micropenis condition as well. The mean age of the group was 18.9, with the youngest man being 12 and the oldest 24 years old. The participants received intramuscular injections of 1,500 IU to 2,000 IU hCG (Human chorionic gonadotropin hormone) on a 3 times per week basis, for a total of 2 months.
Penile lengths (flaccid and erect) were taken regularly, both during the trial and four months after the end of the treatment. All penile length measurements were taken by the same doctor. The doctor used a wooden spatula that was pressed against the pubic ramus, depressing the suprapubic pad of fat as much as it was possible to ensure that the whole part of the penis buried in the fat was measured. Foreskin length wasn’t of included.
According to the paper, the mean flaccid penis length increased from 3.39±1.03 cm to 5.14±1.39, whereas the stretched mean penis length increased from 5.41±1.43 cm to 7.45±1.70 cm. These results can also be seen in the graph below.

A reader of the site brought to my attention another very interesting study named “Transdermal dihydrotestosterone therapy and its effects on patients with microphallus”. The study was published in 1993 and what I find very interesting is that the treatment protocol involved no injections, no oral administration of drugs, no surgery, no use of penile extenders, no nothing. All patients were evaluated for penile and prostatic growth, pituitary-gonadal axis function, serum sex hormone binding globulin, lipid metabolism, hepatotoxicity, bone age and height velocity. All patients demonstrated growth of the penis during treatment. The mean increase rate (153%) in the first 4 weeks of treatment was higher than that (118%) of the second 4 weeks. Of importance is that responses were noted in 4 patients who had failed testosterone therapy for microphallus. The pituitary-gonadal axis was transiently suppressed during treatment, and serum sex hormone binding globulin and lipid metabolism were transiently affected during treatment. Serum alkaline phosphatase increased, mainly due to change of bone isoenzyme but bone ages and mean height velocity were not significantly affected. In conclusion, transdermal dihydrotestosterone therapy is an effective and relatively safe modality in the treatment of microphallus.”

Hopefully I’m back from being away from this blog. Thai man recently underwent emergency surgery after a back-alley penis enhancement procedure lead to him developing penile cancer, the Bangkok Post reports.
“I had olive oil injected into my penis 19 years ago,” the unidentified patient said. “A friend had done it and told us about it. That friend did injections for me and two others at home.” Three years after the surgery, the patient said that the tip of his penis showed signs of infection so he had a circumcision. However, the infections recurred recently and the patient learned he had cancer.
Doctors in Thailand say they are seeing a trend of men wanting to enhance the size of their penises and seeking the services of illegal medical professionals to do so. The Police General Hospital in Bangkok reports that between 30-40 men come to their hospital each month to treat complications to back alley penis enhancement procedures.

Pol Lt Gen Jongjate Aojanepong, surgeon-general of the Police General Hospital, said it is a myth that penis injections increase organ size.

He said injecting olive oil into a penis cannot enlarge its size. The olive oil, which cannot be absorbed into the body, causes irritation and the body reacts by building connective tissue which can turn malignant.
Pol Lt Gen Jongjate strongly urged men who have had penile injections to consult a doctor as quickly as possible before suffering side-effects.
“There are tens of thousands of men who have undergone the procedure. They should see a doctor, the sooner the better,” he said.
He said the Police General Hospital has set up a special medical unit made up of urinary and cosmetic surgeons to attend to the penile injection cases. Psychologists are also provided.
Dr Surat Kittisupaporn, a urologist, said the substances used for penis injections include olive oil and silicone.
“Olive oil is the most popular because it is cheap and widely available,” he said.
Dr. Surat says that it is not possible to completely correct penile disfigurement, though there are methods to correct them to some degree.
“They won’t get it back 100% but they may get an erection if they did not inject the substance into the ‘core’, he said. “But if it turns out to be cancer, it has to be removed.”

What results can I expect?
Your augmentation will depend on your natural penis size and the volume of filler injected.
Our minimum initial injection volume is 7ml. This is less than volumes reported in the medical literature but, like many things, technique counts. Using smaller cannulas to inject and spreading the filler more evenly makes a big difference. The dermal filler works just like it does on the face and other body parts. It is injected just below the surface of the skin and is naturally integrated into the penile tissue after about 1 week. Up until then, the product can be gently massaged to smooth out any bumps and ensure even coverage.

Preparation
Once you have decided to proceed with CALIBRE treatment, there are a few preparation steps, as follows:
•Hair trimming is good a few days prior to treatment. Please be careful not to cause any nicks or scratches with scissors or electric hair trimmer.
•On the day, please wash the area with soap and water. We will also sterilise the injection site.
•Numbing cream – will be supplied at time of booking or you may purchase this from any chemist (e.g. Emla in a 5g tube). Please massage this into the shaft and base of your penis an hour before your appointment and wrap your penis in plastic film wrap – don’t worry about the head (unless you have specifically arranged to have the glans injected).
•You can apply more numbing cream after 30 min if you have some left. Expect some shrinkage with the numbing cream, this may be more noticeable if you are uncircumcised.
•Please arrive 15 minutes before your treatment time and we can apply some extra numbing cream if you would like.
The Procedure
CALIBRE is performed by a skilled cosmetic injectable physician using a thin, rounded cannula. With use of numbing cream, there is minimal discomfort involved in this well tolerated procedure. The type of dermal filler we use is also laced with lidocaine, which numbs the area instantly.
Our physician will first inject a medication that induces an erection. This enables better placement of filler around the shaft and our physician will mould and shape the product as he injects. We may also inject 2-3ml of filler into the head or glans of the penis, if this is required. The procedure will take between 30-60 mins depending on how much product you have injected.
After the injection is complete, your penis will be wrapped firmly in a stretchy bandage, which we ask you to keep on for 12 hours. It may take an hour or so for your erection to relax.
After The Procedure
Whilst there are minimal complications, you should expect:
•Some bruising and tenderness – this varies a lot from patient to patient. Bruising will often become more apparent a day or 2 after the procedure.
•Swelling can result for a few days and may be quite impressive. Do not be alarmed, this will settle and we will provide you with anti-swelling medication. If swelling remains beyond a few days, please contact our clinic for more anti-swelling medication.
•If you have any significant pain or difficulty passing urine, please call and speak to one of our therapists as soon as possible.
•There is some initial malleability to the product. This can be of benefit in allowing you to gently smooth out any small irregularities. If you want to move product further towards the head, or the base, then you will be able to massage it somewhat in that direction, but please don’t over do it. The dermal filler will integrate with penile tissue after 1 week, so abnormal placement will need to be dissolved and re-injected.
•During the injection we need to be careful to not place too much product towards the head of the penis, especially in uncircumcised men. If you notice excessive product around the head, please try to gently massage this back around the shaft.
•Avoid sex or manual satisfaction for 24hrs. It may be a little tender – let him have the day off. No unprotected anal sex for 1 week. Wash your penis after sex for the first week and at least twice a day for that week.
•Dermal fillers may interact negatively with Vitamin E, anti-inflammatory drugs, St John’s Wort and aspirin, possibly causing bleeding and bruising. Do not take those before, during and after CALIBRE treatment.
•You may drive immediately after treatment and no time off work is required.
It takes a couple weeks for our dermal fillers to stabilise, absorb water and look its best. For this reason, we ask you to come back for a follow-up consultation 4 weeks after treatment. If more product is required to ensure even coverage, this can be injected during your follow-up consultation.
How long with the results last?
The longevity of penis augmentation using dermal fillers depend on a number of factors such as your natural metabolism, exercise and the type of filler used.

Other treatment options?
Surgery – lengthening
Surgery to enlarge the penis has been around for a long time. To lengthen the penis the suspensory ligaments holding it to the base of the pubic bone can be cut, through a small incision at the base of the penis. This treatment has some complications, such as:
•It can cause cause considerable swelling of the foreskin in uncircumcised men, so generally a circumcision is required at the same time. This is obviously painful and causes more down time;
•An ongoing stretching program is required, which if not strictly followed fully for at least 6 weeks, may lead to retraction of the penis making it even shorter than before surgery!;
•By using the foundation of the erectile tissue to become part of the shaft of the penis the penis, it can be destabilised, thus, not point ‘up’ so much when erect or it may point to the side; and
•The penis can only be lengthened using this technique and published results indicate the increase in length is generally only 1-2cm.
Surgery – fat grafting to increase girth
Another, more reliable technique, is dermal-fat grafting. In this surgical procedure a strip of skin with the underlying fat is surgically wrapped around the penis under the skin. However, to do this the penis is effectively “skinned” and re-wrapped. This is a highly invasive procedure with material risk of significant scarring from the harvesting of the skin (usually in the crease where the butt joins the thigh). Other Injectible Options
Other substances are being used for penile injections, including: polymethylmethacrylate or PMMA (permanent plastic microspheres), silicone, Vaseline and permanent dermal fillers. There are no simple reversal medications for these injections, so the treatment generally requires surgical removal, which rarely return the penis back to normal.

Men in need of penis reconstruction could soon enjoy the benefits of a special ingredient: stem cells. A new study in rats shows that lacing a penis graft with adult stem cells yields better healing and sexual function than using the graft alone. The finding may point the way to improved treatments for a variety of human penile impairments.
Men with penis injuries, deformities, or severe Peyronie’s disease—which causes excessive scarring that can curve or shrink the penis—sometimes need surgery to reconstruct their genitalia and restore their sexual function. Many receive a graft made of their own tissue, cadaver tissue, or pig intestines, but the surgery can cause complications, including erectile dysfunction.
Wayne Hellstrom, a urologic surgeon at Tulane University School of Medicine in New Orleans, Louisiana, who regularly treats men with Peyronie’s disease and other penile problems, wanted to offer his patients a surgical intervention with fewer side effects. So he teamed up with colleagues in California and China to build a better penis graft.
The team seeded pig intestine grafts with adult stem cells taken from fat tissue in rats. Because rats don’t develop Peyronie’s disease, the researchers had to induce scarring by making incisions in the rodent penises. They then operated on the scarred rats, removing part of the scar tissue and supplanting it with a graft, as is done in patients with severe Peyronie’s disease. Eight rats received the stem cell-laced grafts; another eight got the grafts sans stem cells. A third group had a “sham” surgery that didn’t involve a graft, and a control group didn’t undergo surgery at all.
Eight weeks later, when the rats had recovered from surgery, the researchers examined the rodents’ penises. They found that rats with stem cell-laden grafts had less scarring and better erectile responses, as measured by the animals’ reactions to electric stimulation, than did those with stem cell-free grafts. The rodents’ erections were comparable in rigidity, blood flow, and response time to those in the “sham” and control rats. Cells from the penises with stem cell-laced grafts contained more neuronal and endothelial NOS, enzymes that help trigger and maintain erections. They also had more VEGF, a growth factor whose functions include stimulating new blood vessel development.
The results, published online today in the Proceedings of the National Academy of Sciences, suggest that lacing the grafts with stem cells enhances blood flow and boosts the production of molecules that make and maintain erections, all of which makes for a better penis reconstruction.
“We were really excited and surprised that stem cells can create this much improvement,” says Tulane urologist Asim Abdel-Mageed, a co-author of the study.
“This is an excellent study, with lots of clinical implications,” says Trinity Bivalacqua, a urologist at the Johns Hopkins Medical Institutions in Baltimore, Maryland. But he notes that the method applies only to reconstructions, not to cases where a surgeon may need to build a new penis from scratch. And although the technique works well in rats, “this doesn’t always translate to humans,” Bivalacqua says.
Hellstrom and colleagues plan to test the method in primates next and then eventually in people. “Peyronie’s affects 3% to 9% of adult males and causes a lot of psychological distress,” Hellstrom says. “If we can improve what we have now, it seems like the logical thing to do.”

A double layer of the sheets can be used for maximum widening. The incisions made to perform this surgery are minor and heal quickly, requiring a downtime of only one to two days.

Gathered around an enclosure at the Wake Forest Institute for Regenerative Medicine in North Carolina in 2008, Anthony Atala and his colleagues watched anxiously to see if two rabbits would have sex. The suspense was short-lived: within a minute of being put together, the male mounted the female and successfully mated.
While it’s not clear what the rabbits made of the moment, for Atala it was definitely special. It was proof that a concept he’d been working on since 1992 – that penises could be grown in a laboratory and transplanted to humans – was theoretically possible. The male rabbit was one of 12 for which he had bioengineered a penis; all tried to mate; in eight there was proof of ejaculation; four went on to produce offspring.
The media’s coverage of Atala’s announcement a year later was understandably excited. Not just because of the novelty of a man growing penises in a laboratory, but because his work would fulfil a real need for men who have lost their penis through genital defects, traumatic injury, surgery for aggressive penile cancer, or even jilted lovers exacting revenge.
At present, the only treatment option for these men is to have a penis constructed with skin and muscle from their thigh or forearm. Sexual function can be restored with a penile prosthetic placed inside. The prosthetics can be either malleable rods, with the penis left in a permanently semi-rigid state and thus difficult to conceal, or inflatable rods, which have a saline pump housed in the scrotum. Both technologies have been around since the 1970s. The aesthetics are crude and penetration is awkward.
Another option is a penis transplant from another individual, but this carries a risk of immunological rejection. The chance of organ death can be lessened with anti-rejection drugs, but these drugs have serious side-effects. Transplants can also have a psychological impact, especially with an organ as intimate as the penis. In 2006, a Chinese man was the first to receive a donor penis; two weeks after the 15-hour operation, surgeons removed the transplanted organ on the request of both the patient and his partner.
Atala hopes his technique will mitigate both immunological and psychological issues because his penises would be engineered using a patient’s own cells. “The phallus is actually much longer than you think,” he explains. “It goes all the way behind the pelvis, so no matter the extent of the damage, there is a high probability that there are salvageable cells.”
Peruvian-born Atala, a urological surgeon and professor of regenerative medicine, heads a 300-strong team at the institute. He corrects himself constantly, always going back to edit his speech, adding words such as “high probability“ or “in all likelihood” to be sure his sentences are word-perfect. Soft-spoken and mild-mannered, Atala is a trailblazer in the field and you can’t help but think that his measured speech is an attempt to provide a sure path for others to follow.
To some, engineering human organs sounds like science fiction, but for Atala it’s an absolute necessity. As we live longer (and thus our organs fail more) the shortage of organs for donation will only get worse. If he can work out how to generate the organs people need in a reliable and effective way, the technology can improve a lot of people’s lives. In 2006, Atala and his team announced the first successful bioengineered organ transplant, a bladder, which had been implanted into seven patients in 1999.

Does size matter?
Can a man get bigger and could he do it via stem cells?
Today’s post is the 2nd in a series on stem cells being sold allegedly for a better sex life. You can read the first post here.
An increasing number of men around the world are paying a lot of money for stem cell “enhancement” procedures. Such a procedure could put their health in danger as they strive to be bigger in that area. Men are also more often writing in to this blog asking about this topic, weighing risks to potential benefits.
For example, this message I received from a guy that I’ll call Ted, who said I could post it if I changed his name, tells the story pretty clearly:
“This is embarrassing. I’m having increasing issues with my wife about sex. She’s not a bad person, but she wishes I had a bigger package and could last longer. Can a guy get a bigger penis using stem cells? I’ve seen some ads on the web about this, but it is thousands of dollars and I don’t like the idea of someone injecting me there. Sorry for asking. I feel like an idiot.”
Apparently, at least argued in one recent report, for some women penis size does matter. Another study reported data on the average penis size amongst something like 15,000 men who were measured and the authors suggested guidelines for size related to people who might be candidates for augmentation. For those men who might feel like their size is not where they’d like on those graphs in the paper, one might ask: Can a man get a bigger penis via stem cells?
From Beverly Hills to Bangkok for-profit clinics say an emphatic, “yes!”
Bigger penis. Bigger erection. Better sex life. All through stem cells. And stem cell sex-related “treatments” for women too. If you have the cash to pay us.
Would this kind of stuff really work?
If you’ve got the necessary bank account to pay for it and can overcome the scary idea of injections or other procedures on your genitals, you could try it, but there are very serious potential risks and little published data to support doing something rather extreme like this. It would well make things worse instead of better. Such risks could include complete loss of sexual function, nerve damage, infection, and more.
From a broader perspective, stem cells are being sold to allegedly grow or improve all kinds of body parts including not only penises, but also breasts whether surgically or via that odd electric stem cell bra. Stem cells are also being sold to “rejuvenate” vaginas in women. What the heck?
It is important to distinguish these kinds of cosmetic procedures from breast or other tissue reconstruction after cancer surgery and also efforts to regrow penises damaged, for example, by battlefield injuries to male soldiers.
As I said, I’ve gotten a significant number of emails and comments on the blog from men like Ted who are seriously contemplating getting a “stem cell” enhancement and from women looking to deal with sexual dysfunction. It’s no April Fools Day joke to them. Some of the men want to be bigger and some have erectile dysfunction.
Who is selling this stuff?
There are many places, but I’m going to give two examples that are certainly not endorsements or specific criticisms. To be clear again, I discourage people generally from going this route because of serious safety concerns with such procedures.
The Stem Cell Rejuvenation Center makes a claim of selling better sex for $1,500 and up a pop via stem cells by procedures they call O-Shot for Women and Male-Shot. How stem cells could achieve their claims (listed below in red) is not clear to me and any kind of procedure is going to have risks:

◾Better sex after menopause
◾Better sex after childbirth

◾Increased sensitivity

I didn’t see any data on this to support these claims.
Another place called the Barron Centers in Beverly Hills offers stem cell-based phalloplasty.
On the Barron website they describe in great detail how the attempts to use fat or stem cells to get bigger penises has evolved over the years to get to today’s technology (emphasis mine):
“Fat is removed and placed in the penis the same as before, but it is processed in a way that separates stem cells from the rest of the fat, which are then injected into the fat in a very high concentration. A new blood supply is established rapidly which allows for a much better result. There is a small amount of reabsorption, but nothing compared to what we used to experience. There will be an increase in circumference of 30-40% either way.”bigger penis stem cells

The doctor behind the Barron Centers is Dr. Rodney Barron. The idea seems to be to use fat tissue to increase size and stem cells to “grow” more tissue as well as stimulate increased vasculature.
It is important to note that the laboratory process of the material that they describe seems to me to be production of a specific stem cell-enhanced adipose product called SVF or stromal vascular fraction (more on that here). This is notable because the FDA to date has generally written about SVF as a drug requiring pre-approval and licensing.
The bottom line overall when it comes to stem cells for penile enlargement or even for a better sex life more generally is that it seems like an extreme way to go without clear data supporting efficacy and safety. There are likely to be some serious medical risks involved.
Enjoy a Bigger & Harder Penis the Day of Treatment!
When I first started doing Cosmetic procedures using Juvederm and Restylane to sculpt the face, it occurred to me that it would be wonderful to do the same thing for the penis. If you can inject the mouth to give it a larger shape then why should you not be able to do the same thing with the penis? Here’s the web site where I talk about my skills with that procedure.
Then Restylane came out with their research showing that after filling in the face, that there is new collagen production and the face gains permanent improvement in shape as dictated by the injection.
I became even more interested and considered injecting my own penis with Restylane.
Here’s the potential complications of the hyaluronic acid fillers (Restylane and Juvederm):
1.With Restylane you can, especially in the forehead, have the side effect of the skin necrosis (it turns black and dies and must be surgically removed)! Not a pleasant thought when considering part of the skin of the face, so I never inject the forehead with Restylane where this is known to occur. But, even more disturbing it is to think of necrosis of the penis.
2.There can be some burning pain with injection of Restylane (it is hyaluronic acid). OK, I don’t like admitting it but I’m not a big fan of pain, especially when it comes to my penis. Not very brave I suppose but I’d much rather have you punch me in the nose than give me a penis ache.
3.Lumpiness and nodules can occur with Juvederm and Restylane. Not common in my practice. But, I discontinued the use of another hyaluronic acid filler, Hydrelle, because I did see a higher than expected rate of nodule formation. In the face, a nodule is bad enough but in the penis, I just did not want to accept the possibility.
So, I never injected my penis with Restylane.
The Answer Knocks on My Door
After studying the biology of platelet-rich plasma (PRP), I invented a way to rejuvenate the face using this natural product (from the person’s own body). I called it the Vampire Facelift ® and registered it with the US Patent & Trademark Office.
Here’s what I found when using PRP in the face:
1.There’s never been an allergic reaction or necrosis (this is coming from your own body, so the body does not react to it as a foreign substance).
2.Bruising is minimal if at all.

What the Research Shows
So, then I went to the medical literature, to see what other experts around the world are thinking, and here’s some of what I found:
•When PRP is injected into nasolabial folds with improvement seen immediately. There was some fading of the improvement over the first few days, then the tissue continued to regenerate and return to near the state of injection by the 12th week. At no point did the tissue go back to the state before injection. None of the patients had any fibrosis, harness, lumpiness, irregularity, or restriction of movement. There have been over 50,000 injections of Selphyl and never one allergic reaction.
•Researchers showed an increase in healing of wounds using PRP combined with an HA filler.
•Using that technique, I rejuvenated a woman’s ankle
•In January of (2010), researchers at the University of California School of Medicine injected adipose-tissue derived stem cells into the penis of diabetic rats. The result was that the rats. The results were to “improve erectile function and to alter the micro architecture of the corpus cavernosum with an “increase in the number of endothelial cells” in the corpus cavernosum. Translation: the rats had a harder penis with more tissue–harder and bigger! Notice: In the article, they report that the number of stem cells was so small that they think that the results came from the “paracrine” effects of the fat. This means that they thought the growth factors in the fat are what caused the improvement. These are the same types of growth factors found in huge quantities in the PRFM of Selphyl
•Just a little reading shows that PRFM is being considered for many diseases that require healing–as in NEW TISSUE GROWTH–that’s what healing is. Sure, insurance is not paying yet, but I hope that you realize the conflict of interest when insurance agencies are required to make a profit (by corporate law) while simultaneously appearing to care about your health.
•As early as 2003, an article out of Boston in the Journal of Urology, discussing new therapies for erectile dysfunction, suggested that “neovascularization using vascular growth factors has been recently demonstrated to be feasible in animal models.” Translation: If you inject growth factors into an animal’s penis, then it grows new blood vessels and new tissue and gets a bigger harder penis. So, 7 years ago, this was suggested as a possibility for people-animals. PRFM is a way of supplying those growth factors.
If you “Goggle” platelet rich fibrin matrix or stem cells and healing, you will be amazed that this therapy is not used more often for a wide variety of problems.
IMPORTANT: The woman who’s breast I injected, said that she got renewed sensation of the nipple (she had suffered with decreased sensation after breast implants followed by breast feeding 3 children). With the neovascularization (new blood vessels), there also appears to be some nerve regeneration. As with the diabetic rats in the study quoted above, for the man who has diabetes and erectile dysfunction with some decreased sensation and nerve damage, Selphyl could be a near miracle.

Question: I did some research and most people say it takes three weeks to notice results but you said you saw it immediately. Please explain? Could it be that everyone reacts differently to it? thanks and talk soon.
Answer: If you read the numbers on the scale in the Selphyl research that I posted above, then you will see something similar to what I have observed. The initial volume is from the combination of the platelets and the serum (or water) in the blood plus all the growth factors in the water that the activated platelets are starting to release.
Then over the next three days some of the the water is absorbed by the body. Then there is the growth of new tissue that continues for at least 12 weeks–bringing the volume back up to close to where it was with the initial injection.
So, volume goes up IMMEDIATELY, then goes down to various degrees but slowly over the next three days. In some people this decrease in volume is very small, in others it is a more noticeable decrease. In some, I’m seeing a growth back to where it was with the initial injection or better. In others, there is growth but not quite back to where we started. Smokers have the least amount of growth.
Again, I am speaking mostly from my experience with injecting the face. I see no reason why the same would not happen with the penis, but my experience there is less.
Question: How much growth can I expect?
Answer: 10 to 20% increase in length and girth. An inch in length (a little more) and in girth was seen in the first two patients in the first month. Girth came first (with the injection), then length (with continued growth). I do not yet know if a second injection at 4 to 8 weeks will cause another growth spurt.
Question: Where is the injection put?
Answer: Directly into the corpus cavernosum with a small needle, and into the head of the penis–in a way almost exactly as done with those who use injections for erectile dysfunction.
Question: What about scar tissue?
Answer: If you look at men who inject 3 to 5 times a week over many years for treatment for erectile dysfunction, there is about a 10% incidence of scar tissue formation. This is with multiple injections for years. With this procedure, there is only one injection, so the chances of scar tissue should be almost nothing. In the face, there has not been the report of any fibrosis or scar tissue formation.
Question: Does it hurt?
Answer: I use a lidocaine cream (a strong version of what you might find in a spray to kill the pain of sunburn). It’s the same cream I use on the face before injecting the lips or other areas with Juvederm. The pain is about the same as a normal shot to the buttocks, perhaps less.
